It's just that when you're on foot, with options set to "mouse + keyboard", the camera no longer automatically swings around to face the same way as C.J.
(Actually, you can't use the mouse to look around at all, even when driving a vehicle or bike. is out walking around, you cannot use the mouse to look around. The problem is that in the menus, the left mouse button (LMB) could not be used to click on the menu options. I recently dug up my copy of Grand Theft Auto: San Andreas (Second Edition) and installed it. This drove me up the wall for a while tonight.
